Transition planning is an important process for building a path to adult life.
This training focuses on the definition, aim, and design of an Individual Transition Plan (ITP) in the IEP.

Starting with the assessment process, linking to self-determination, considering meaningful goals, and providing corresponding services while bringing together mandates of various agencies are the foundations of the ITP.
